Effective 2026. Hickory Weight Loss is a telehealth service operated by Oakwell MD ("we", "us"). This policy covers weightlosshickory.com and the online questionnaire on it.
Information we collect
- Website data: pages viewed, referring site, approximate device and browser details, and how you interact with the site, collected through cookies and similar technologies.
- Questionnaire answers: the goals, preferences and contact details you choose to enter when you tap Get Started.
- Health information: once you become a patient, the health history, medications and messages you share with the care team. This information is protected health information under HIPAA and is handled under our Notice of Privacy Practices.
How we use it
To review your eligibility for treatment, deliver care and medication, respond to your questions, improve this website, and send you updates about your plan. We do not sell personal information.
Sharing
We share information only with the licensed providers and pharmacies involved in your care, service providers who operate this site under confidentiality agreements, and where the law requires it.
Your choices
You may request a copy of your information, ask us to correct it, or ask us to delete it where the law allows, by emailing [email protected]. You can opt out of marketing email at any time using the link in each message.
Security
We use encryption in transit and at rest, access controls and audit logging. No system is perfectly secure; if we learn of a breach affecting your information we will notify you as required by law.
Children
This service is for adults 18 and over. We do not knowingly collect information from children.
